A 45 year old man fell heavily on the stairs six months earlier. His previous history included a motor vehicle accident 20 years earlier that led to two spinal operations including laminectomy at L5/S1 and a fusion. His pain score is now 8-9/10 and he has substantial morning stiffness. This patient has already had sacroiliac joint blocks which proved to be negative. This video clip presents a lumbar medial branch block. The only way to determine whether the facet joints are the cause of pain is to either inject the joint or its nerve supply with local anesthetic in order to ascertain whether or not the pain is eliminated.

A discussion of back pain management in the post-surgical spine is presented, along with a more detailed discussion of the pain nerve supply to the facet joints.
