A man in his 50s is presented with chronic low back pain on the right. His pain is worse on stairs and he finds it hard to get in and out of his car or sit in a soft chair.

Medial branch blocks more than halved his pain taking it from a pain score greater than 6/10 down to 2/10. The right L3/4, L4/5 and L5/S1 facet joints are de-nervated by radiofrequency neurotomy.
