Caudal epidurals are where the injection is given in the lower sacrum (tail bone) and the fluid travels up the spinal canal.
Transforaminal Epidural Steroid Injection is new more precise technique which has been developed with the assistance of C-arm fluoroscopy (a high tech x-ray machine that looks at the spine from all directions). In this instance the needle is aimed under x-ray guidance directly between the nerve root and the corner of the disc. The main advantage of this technique is providing a greater concentration of therapeutic agent at the site of inflammation.
Both types of epidurals are demonstrated in a selection of the clips found in this section.
A 47 year old man with disc bulging adjacent to the L5 nerve root causing significant right leg ‘sciatica’ referred pain is treated with an L5 transforaminal epidural steroid injection, under live x-ray imaging. .
